Freedom of Information Act (FOIA) Requests 

The Freedom of Information Act 442 of 1976 declares that it shall be the public policy of this state that all persons, except those persons incarcerated in state or local correctional facilities, are entitled to full and complete information regarding the affairs of government and the official acts of those who represent them as public officials and public employees, consistent with this act. The people shall be informed so that they may fully participate in the democratic process. MCL 15.231(1)

If you are requesting a Release of Motor Vehicle Crash Report that has happened within the last 30 days, please complete the UD-9 fillable form and submit with your Freedom of Information Act Request, by email.  This form is required to be completed before the release of any crash report, pursuant to MCL 257.503.  UD-9 Form
